Does the combination beat caffeine on its own — and how would we know?

The combination works. That is the easy part.

A 2025 systematic review searched the literature and found 49 randomized trials, of which 16 could be pooled. Against placebo, theanine plus caffeine came out ahead on attention.1

Two significant results, both small, both in the second hour. The first-hour reaction-time estimate looks large but its confidence interval crosses zero, so it does not count.

An earlier meta-analysis of 11 studies found much the same: moderate effect sizes favouring the combination in the first two hours, on alertness ratings and attention-switching accuracy.2

So the compound stack does something measurable. The interesting question is a different one, and almost nothing in this literature is designed to answer it.

Compared to what?

Caffeine on its own is one of only two compounds this site grades Strong · Supported. It improves attention and vigilance in a literature deep enough to have reviews of its reviews.

Which means the question that matters for anyone buying a theanine-and-caffeine product is not does this beat placebo. It is does this beat the caffeine I was going to have anyway.

Answers: does adding theanine improve on caffeine? This is the question the product is sold on, and it is far less often the question tested.

Check what the comparator was. A trial showing a combination beats placebo tells you the combination works. It tells you nothing about whether the second ingredient earned its place — that requires an arm with the first ingredient alone.

This is a habit worth carrying beyond this page. Any time a stack, blend or formula is shown to beat placebo, the useful follow-up is: was it also compared against its most active single component?

The dose analysis nobody quotes

The 2014 meta-analysis did something the others did not: it looked at whether the size of the effect tracked the theanine dose or the caffeine dose.

Moderator analysis of caffeine and L-theanine doses revealed trends toward greater change in effect size for caffeine dose than for L-theanine dose, particularly during the first hour postdose. Camfield et al., Nutrition Reviews, 20142

The effects moved more with how much caffeine was given than with how much theanine was given. That is a trend rather than a demonstration, and the authors present it as such. But it points the same direction as the comparator problem: much of what the combination does may be what caffeine does.

What theanine does on its own

If theanine were a strong cognitive agent by itself, the question above would matter less. A 2025 meta-analysis pooled five randomized placebo-controlled trials in 148 healthy adults to find out.3

One outcome of four reached significance. The authors put it plainly: the benefits of theanine on cognitive performance "could not be confirmed by all test methods," and further trials with standardised products and larger samples are needed.3

Note the third row. On the harder Stroop condition the point estimate is positive — slower, not faster — by more than a tenth of a second. The interval crosses zero so this is not a finding of harm, and we are not presenting it as one. It is simply not the shape of a result that supports a confident claim in the other direction.

A separate review found theanine alone did improve choice reaction time in the first hour.1 The two analyses disagree, which is why the theanine-alone claim below is graded Mixed rather than Supported.

Where the separation has been attempted

Some trials have included a caffeine-only arm. The most cited gave 27 participants 50 mg caffeine with and without 100 mg theanine, in a crossover with a week between conditions.4

Read side by side, the combination arm did more than the caffeine arm: it moved both speed and accuracy on attention switching, and it reduced susceptibility to distracting information in a memory task at both time points, which caffeine alone did not.

That is genuine evidence of an added contribution. It is also 27 people in a single trial, on a handful of laboratory measures. It supports "there may be something here" rather than "the combination is established as superior to caffeine."

The mood claim is the one most heavily marketed — theanine as the thing that removes caffeine's jitter. The 2025 review found small-to-moderate improvements favouring the combination, but with confidence intervals that often included no difference at all.1 That is not nothing, and it is not established.

Safety and unknowns

  • Adverse events reported

    Neither compound raised safety concerns across the pooled trials. L-theanine is a constituent of tea and was well tolerated at the doses studied. Caffeine's own profile is covered in its separate breakdown, including the EFSA intake thresholds — the caffeine component is where any dose caution belongs, not the theanine.

  • Longest human exposure studied

    These are acute trials — single doses, with outcomes measured over one to three hours.12 Nothing here speaks to taking the combination daily for months, which is how it is typically sold.

  • Known interactions and at-risk groups

    Not characterised for theanine. For the caffeine component, the relevant cautions are pregnancy, existing anxiety conditions and cardiac arrhythmia, and the applicable intake limits are lower than the performance literature's doses.

  • Regulatory status

    Both are sold as dietary supplements; L-theanine occurs naturally in tea. No tolerable upper intake level has been established for theanine.

  • What has not been characterized

    Whether adding theanine improves on caffeine alone, at any dose, in any population — the comparison is rarely the one tested. Effects beyond a few hours. Optimal ratio. And, as the 2025 reviewers note, most studies were conducted in laboratories and few used tea or tea-equivalent doses,1 so how any of this maps onto drinking tea is genuinely unclear.

The headline claim is graded against placebo, because that is what the pooled evidence tests. A reader should hold that in mind: Moderate · Supported here means "better than nothing," not "better than coffee."

Bottom line

Theanine and caffeine together improve attention compared with placebo. The effects are real, small, and show up in the second hour rather than immediately. That much is reasonably well established across two meta-analyses.

What is not established is the thing the product is actually sold on. Almost all of this evidence compares the combination against placebo rather than against caffeine alone — and caffeine alone already carries our highest grade. The one meta-analysis that examined dose found effects tracking the caffeine more than the theanine, and theanine by itself failed to improve three of four cognitive measures in the most recent pooled analysis.

Where a caffeine-only arm has been included, the combination did outperform it on attention switching and resistance to distraction — in twenty-seven people, once. That is a reason to be curious, not a reason to be confident.
